Please download the 2008-2009 IHONC Registration form in either PDF or Excell format and follow the instruction below.

1. If you are a level I or level II USA Hockey Certified official 18 years of age or younger, your registration fee is $15. If you are over the age of 18 or a level III or IV certified official, your registration fee will be $25.

2. You need to print either format of the application, fill it out, sign and mail it with your check payable to IHONC.

3. If you are renewing your membership with IHONC, your application MUST be received by September 30. Those received after the 30th will incur a $10 late charge.
